Living Will Form
I,______________________________, being of sound mind and body at this point in time, I herby wish to make my future wishes known to all. If an unfortunate health issue in the future should incapacitate my mind and body, thereby not allowing me to make my own decisions it should be known to all.
Under no conditions shall I be kept alive by some by artificial means. My fate shall not be decided by some Pinheads, Hospitals, Doctors or Lawyers that wish to run up a medical bill and keep me in a vegetative state that is foreign to my outgoing lifestyle.
If after a reasonable amount of time, I have not requested any of the following items; it can then be safely assumed that I will not be coming around to tell tall stories anymore.

- _____A little fun in the sack
Send out the appropriate news to the population with instructions to BYOB for the tailgating parking lot meet and greet, after paying the proper respects.
